Opting out of NHS Digital collecting your data (Type 1 Opt-out) – IMPORTANT PATIENT INFORMATION

The deadline for patients to opt out has been extended and we are awaiting further information on the new date. However we recommend you registered your opt out preferences without delay.

Data sharing with NHS Digital will start in September 2021 (delayed from 1st July 2021)

If you wish to register a Type 1 Opt-out with your GP practice before data sharing starts with NHS Digital, this should be done by returning this form to your GP practice before 25th August 2021 to allow time for processing it. If you have previously registered a Type 1 Opt-out and you would like to withdraw this, you can also use the form to do this. You can send the form by post to Cuckfield Medical Practice, Glebe Rd, Cuckfield, RH17 5BQ or by email to the practice cuckfield.reception@nhs.net or call 0300 3035678 for a form to be sent out to you. Please DO NOT call the practice for a form.

Paper copies of this form will also be available in the lobby areas at both sites with a box for patients to put their completed forms into. Our reception staff will empty the boxes daily and add the opt out code to patient records accordingly.
