Cuckfield Medical Practice is part of a Primary Care Network, called Haywards Heath Villages, with Lindfield Surgery and Ouse Valley Practice. Together our PCN covers around 30,000 registered patients. We work collaboratively with our PCN member practices to provide some services to our patients. Our PCN has recently employed two Pharmacists, and a Social Prescriber and we have more plans in the new year to increase that team. We aim to work more closely with our community services, health visitors, midwives, community nurses etc to provide a more integrated service for all our patients.
